The Office of Student Conduct makes available online a form for reporting incidents involving students. While primarily designed for residential staff (i.e., Resident Assistants, Graduate Residents, and Residence Coordinators), anyone with a Duke NetID may use this form to report an incident involving undergraduate students.
Filing a Report
Upon receiving a report, Office of Student Conduct staff and/or Housing, Dining, and Residence Life (HDRL) staff will respond as appropriate. You will be asked to authenticate your identity before being directed to the online incident report form. If you have any problems accessing the form or authenticating your identity, please contact us.
If you are not a member of the Duke University community or do not have a NetID, you may report incidents involving undergraduate students by emailing details to email@example.com. Any individual who wishes to ensure that his/her report is only viewed by the Office of Student Conduct should email firstname.lastname@example.org in lieu of completing the Web form. If you wish to file a report anonymously, you may do so through DukeReach by clicking here, though Duke encourages individuals who are reporting a crime to provide identifying information.
